Eligibility
- Must be 60 years old and above.
- Born in Selangor or Wilayah Persekutuan.
> Selangor: middle number in your IC shows the number 10.
> Wilayah Persekutuan: middle number in your IC shows the number 14.
...eg: xxxxxx-10-xxxx / xxxxxx-14-xxxx - Been living in Selangor for more than 15 years.
- Passed away after 1 October 2008.
Unfortunately, those who have passed away before then are not eligible.
Application
- Come to our service centre to obtain and fill in the Mesra Usia Emas forms.
(If you've already filled in the form that's without a serial number, do come to our service centre to fill in a new form. The old forms were sold by various people so the new form has a "percuma" clause on top so that it can't be sold.) - Bring along:
- two photocopy ICs of applicant
- two photocopy ICs of beneficiary. - If the applicant was not born in Selangor/Wilayah Persekutuan but has been living in Selangor for more than 15 years, do bring documentation as proof:
...eg: child's birth cert (if child is over 15 years old and was born in Selangor/Wilayah Persekutuan), land title, marriage certificate (if married in Selangor/Wilayah Persekutuan more than 15 years ago), etc.
(Applications without these documents will not be proccessed as this scheme is meant for constituents within Selangor only.) - If applicant is unable to pen a signature, their left thumb print is required on all three sheets of the Mesra Usia Emas form. (One copy for the State Treasury, one copy for our reference, one copy as your receipt.)
- According to your IC address/constituency, submit your forms to your respective ADUNs. (There are no virtual forms as these forms have carbon copies for us to keep as records.)
> Constituents whose IC addresses are within these sections are able to submit their forms to the Bukit Gasing service centre: Sections 1, 1A, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 16, 17, 18 and 52. - If your address is under Wilayah Persekutuan, do submit form(s) to your respective parliamentarians.
- Applicant need not submit the form themself. Just as long as their signature/left thumbprint is on the application form, someone else may submit on their behalf.
- Applicant's beneficiary will be given monetary aid of rm2,500 upon death to help bear funeral expenses.
- After being registered, the beneficiary will be able to collect rm1,000 immediately from our service centre. The remaining rm1,500 will be dispersed within 60 days by the District Office.
- Bring along: death certificate, police report, and burial certificate.
- If the beneficiary is unable to collect, the beneficiary must write a supporting letter stating the reason for not being able to and also name the person who'll be collecting on his/her behalf.
